Few providers publish prices for genital surgery; most quote after a consultation or written enquiry.
Plan for two to four weeks or more in Thailand after vaginoplasty, depending on the provider and technique.
Our method for this page
We read each provider's own web pages on gender-affirming surgery clinics in Thailand and kept only statements we could trace to a source. Nobody paid to be included, and the list is not a quality ranking. Criteria:
Bumrungrad's Pride Clinic, on the 16th floor of Building A, coordinates care for transgender adults and young people. For trans women it lists facial feminisation, tracheal shave, breast augmentation, vaginoplasty and orchiectomy; for trans men, mastectomy, phalloplasty, metoidioplasty and laparoscopic hysterectomy with oophorectomy. Voice surgery is listed for both.
The clinic names four plastic surgeons alongside an endocrinologist, gynaecologists and a child and adolescent psychiatrist, and offers hormone treatment and help with updating identity documents. It says surgery follows a pre-operative evaluation of physical fitness and of the patient's understanding of the risks and benefits, but publishes no further eligibility criteria and no surgery prices.
It may suit people who want hormone care, surgery and other specialists in one large international hospital.

Bangkok Hospital's Plastic Surgery and Reconstruction Center publishes a guide to gender-affirming surgery written by two of its plastic surgeons and updated in September 2026. For trans women it describes penile skin inversion as the usual first operation, with rectosigmoid and peritoneal vaginoplasty as alternatives; for trans men it covers mastectomy, hysterectomy with oophorectomy, vaginectomy, metoidioplasty and phalloplasty.
Its stated requirements are stricter than WPATH SOC8: assessment by at least two psychiatrists with medical certificates, living full-time in the affirmed gender for at least a year, and an age over 20 (18–20 with parental consent). Metoidioplasty requires one to two years of testosterone beforehand.
No prices for these operations are published. It may suit people who want genital or chest surgery within a large hospital with intensive care on site.

It asks for a formal diagnosis of gender dysphoria from a licensed psychiatrist and hormone therapy for a period set by the patient's own doctor, and advises stopping smoking and alcohol for at least a month before surgery. It does not name surgeons or publish hospital stays or prices.
It may suit people who want their assessment, hormone care and surgery coordinated within one private hospital in Bangkok.

The Phuket Plastic Surgery Institute (PPSI), based at Bangkok Hospital Siriroj in Phuket Town, publishes a male-to-female surgery page naming Dr Sanguan Kunaporn as the surgeon. It describes penile skin inversion combined with a full-thickness scrotal skin graft, and colon vaginoplasty for patients who need more depth.
Patients must be at least 18 (under 20 with parental permission) and need approval and a referral letter from a psychiatrist, psychiatric social worker or clinical psychologist; foreign patients also need a letter from a GP or psychologist. The page does not cite WPATH. Hormones are stopped at least 14 days before surgery, stitches come out after seven to ten days, and patients stay in a hotel after discharge.
The package starts at 560,000 THB. It may suit trans women who would prefer to recover in Phuket rather than Bangkok.

Kamol Cosmetic Hospital in Wang Thonglang publishes five vaginoplasty techniques, from zero-depth and penile skin inversion to sigmoid colon (open or laparoscopic) and penile–peritoneal vaginoplasty, along with revision surgery, facial and voice feminisation, phalloplasty using several flap techniques, metoidioplasty and top surgery. Its founder, Dr Kamol Pansritum, is described as having carried out more than 5,000 male-to-female procedures since 1997.
The hospital's own requirements are stricter than WPATH SOC8 and do not cite a version: patients should be at least 20 (18–20 with parental or guardian consent), have had at least one year of continuous hormone therapy, and be assessed by two Thai psychiatrists. Some techniques also carry BMI and weight limits.
It advises planning about four weeks in Bangkok, including four to seven nights in hospital depending on the technique, and offers on-site serviced apartments. It may suit people wanting a wide choice of techniques, including phalloplasty, in a dedicated surgical hospital.

For trans women it publishes penile skin inversion (with or without a scrotal skin graft), sigmoid colon and zero-depth vaginoplasty and orchiectomy, plus facial feminisation; for trans men it lists mastectomy, metoidioplasty, hysterectomy with oophorectomy and vaginectomy. It does not list phalloplasty.
Its vaginoplasty page says the clinic follows WPATH standards without naming a version, and asks for two letters from qualified mental health professionals, at least 12 months of continuous hormone therapy unless contraindicated or declined, a minimum age of 18 and experience living in the affirmed gender role. The package includes five nights in a private room at a JCI-accredited partner hospital, and the clinic asks patients to plan on about 18 days in Bangkok.

How to compare gender-affirming surgery clinics in Thailand
Thailand has a long record in gender-affirming surgery, and people travel from many countries for vaginoplasty, facial feminisation, chest surgery and, increasingly, phalloplasty and metoidioplasty. Providers range from specialist clinics led by one surgeon to large private hospitals with a multidisciplinary team. The surgeon, the technique and the plan for recovery and lifelong aftercare matter far more than the price.
This guide is general information, not medical advice. Only a surgeon who has assessed you can say which procedure and technique suit you, and every operation carries risks.
Types of provider
Specialist gender-affirming surgery clinics and dedicated surgical hospitals in Bangkok often focus on a small number of procedures that their surgeons perform frequently, and may operate at a partner hospital. Large private hospitals offer surgery through plastic surgery departments or LGBTQ+ clinics, with endocrinology, gynaecology, urology and psychiatry in the same building.
Neither model is better in itself. What counts is the experience of the named surgeon with your specific procedure, what happens if you need intensive care or a second operation, and how the provider supports you after you go home.
Who is the operating surgeon, and how many of this exact procedure do they perform each year?
Where will the operation take place, and is that facility licensed and equipped for complications?
Is there a urologist, colorectal surgeon or microsurgeon available if your technique needs one?
Assessment letters and WPATH SOC8
The World Professional Association for Transgender Health (WPATH) published Version 8 of its Standards of Care (SOC8) in 2022. SOC8 recommends that adults need a single opinion from a professional competent in assessing transgender and gender diverse people before gender-affirming medical or surgical treatment, replacing the two assessments that earlier versions required for genital surgery. Further opinions may be requested where there is a specific clinical need.
SOC8 also suggests considering at least six months of hormone therapy before gonadectomy and some genital procedures, unless hormones are not wanted or not medically suitable. Thai providers set their own rules, and several publish requirements stricter than SOC8: two letters or two psychiatrists, 12 months of hormone therapy, a period of living in the affirmed gender, or a minimum age of 20 with parental consent from 18. Some still refer to older versions of the standards.
Read the provider's requirements carefully and get them confirmed in writing before you book flights. Some clinics can arrange a psychiatric assessment in Thailand for a fee.
Choosing a technique
For vaginoplasty, providers may offer penile skin inversion (often with a scrotal skin graft), techniques that use a segment of colon or the lining of the abdomen (peritoneum), or zero-depth vaginoplasty. For trans men and non-binary people, options include mastectomy (top surgery), hysterectomy with oophorectomy, metoidioplasty and phalloplasty, which is usually staged and uses tissue from the forearm, thigh or elsewhere.
Each technique differs in recovery time, risks, need for dilation, and suitability for your body. Some providers set BMI or weight limits for particular techniques. Ask the surgeon to explain why they recommend one technique for you, and what the alternatives are.
Costs and what packages include
Few providers publish prices for genital surgery; most quote on request after reviewing your details. Packages usually cover the surgeon, anaesthetist, operating theatre, a set number of nights in hospital and basic medicines, and may include airport transfers or a first set of dilators.
Common extras include hotel nights after discharge, a psychiatric assessment in Thailand, cardiology clearance for older patients, revision surgery, extra charges for some medical conditions and credit card fees. Ask for a written quote that lists exclusions, and keep a contingency for a longer stay.
Recovery and time in Thailand
Providers commonly ask you to stay in Thailand for two to four weeks after vaginoplasty, and sometimes longer, so that stitches can be removed and dilation can be checked. Phalloplasty usually involves more than one operation and a longer overall timeline. Surgery and long flights both raise the risk of blood clots, so ask your surgeon when it is safe to fly.
Vaginoplasty needs regular dilation for many months. Plan for rest, help at home and time off work, and make sure you understand the dilation schedule before you leave.
Aftercare once you are home
Arrange a local doctor, ideally one with experience of post-operative care after gender-affirming surgery, before you travel. Ask the provider for your operation notes and discharge summary in English, and for a way to send photos or questions to the surgical team.
Find out how the provider handles complications that appear after you have left Thailand, whether revision surgery is covered, and who pays for return travel if it is needed.
Questions to ask before you book
Who will perform my operation, and how many of these procedures have they done?
Which technique do you recommend for me, and why?
Exactly which assessment letters do you need, from whom, and must they follow a particular version of the WPATH Standards of Care?
How long must I have been taking hormones, and should I stop them before surgery?
Are there BMI, weight, age or health limits for this technique?
What does the quoted price include and exclude, including hotel nights and dilators?
How many nights will I spend in hospital, and how long should I stay in Thailand afterwards?
Where will the surgery take place, and what happens if I need intensive care?
What is your policy on revision surgery, and who pays for it?
How do I contact the team after I return home, and will I receive operation notes in English?
When will it be safe for me to fly?
Warning signs
No named surgeon, or a surgeon whose licence you cannot check with the Medical Council of Thailand.
Requirements that change between conversations or that conflict on the provider's own website and are not confirmed in writing.
A quote with no list of what is excluded, or pressure to pay a large deposit before a consultation.
A recommended stay in Thailand that seems too short for the technique, or no plan for stitch removal and dilation checks.
Guarantees of particular results, such as an exact depth or sensation.
No clear arrangement for aftercare or complications once you are home.
How many assessment letters do I need for surgery in Thailand?
It depends on the provider. WPATH SOC8 recommends a single opinion from a qualified professional for adults, but several Thai clinics and hospitals still ask for two letters or an assessment by two psychiatrists. Confirm the requirement in writing before booking.
How long do I need to stay in Thailand after vaginoplasty?
Published guidance varies by provider and technique, from around two and a half weeks to about four weeks, including several nights in hospital. Phalloplasty usually involves more than one stage. Ask your surgeon for a timeline and when it is safe to fly.
What is the minimum age for gender-affirming surgery in Thailand?
Providers set their own limits. Several publish a minimum age of 20 and accept 18- to 20-year-olds only with parental or guardian consent; at least one lists 18 as its minimum. Confirm the rule with your provider.
Are prices published?
Rarely for genital surgery. One provider publishes a starting package price; others quote on request. Ask what is excluded, such as hotel nights, assessments in Thailand, revision surgery and card fees.
